Duplicate Professional Certificate (Proof of Certification)

This item is to get a digital copy of your Official Professional Certification from a BirthWorks program you already completed in the past. Please review the following:

1. If you were certified within the last two years, the expiration date is two years after your final official certification date.
2. If you recertified after two years (and every two years since), the future expiration date is shown (always in two-year increments after your first official certification date).
3. If you did not recertify, it will show the past expiration date (exactly two years after your final official certification date).
4. If you are not certain about your recertification status or need to recertify (before making this request), more information on recertification is here.

If you were certified in more than one program, choose the number of Professional Certificates you want to request by adding more than one of this item to the cart and then email the details of what you need to info@birthworks.org right after you make payment.
