In this step, we ask that you verify your commitment to The BirthWorks Philosophy and The BirthWorks Approach.

All BirthWorks Certified Professionals – CCE(BWI), CBD(BWI), CPD(BWI) – must be current BWI Members in good standing to continue to use their credentials and say they are BWI Certified. They must also confirm the following:

(1) Alignment with the Vision & Mission and the BWI Philosophy (specifically the Statement of Beliefs).

(2) Agreement to the expectations and requirements described in the Profile of a BirthWorks Professional; Ideals of a BirthWorks Professional; Professional Standards of Practice; Scope of Practice; S.U.P.P.O.R.T.; Privacy Guidelines, HIPAA & Client Confidentiality; and to follow the Policies.

What you see to the right is a sample Verification Agreement. Each document listed is (1) in your Certification Packet and (2) part of this online training. If you still have questions or need clarification, please ask your Mentor before you sign.

When ready, go to the above Materials tab, to download the Verification Agreement to sign. (There are two versions – choose either 8×11 or A4 size to download and print).

After you sign and date it, please scan it and upload it below.
